Welcome to KnoWEwell!  You will be prompted to complete your Provider Profile forms after you create your KnoWEwell account.  It is very important to complete the Profile so members of the KnoWEwell Community can search for and find you with our unique matching tools.  The information you provide in this form will automatically populate your Profile in the KnoWEwell Community! Once you submit your forms, the information will be verified and an invitation to Activate your Membership will be sent to your email. 

IMPORTANT:  You must SAVE your forms if you are not completing them and consenting in one session. You will be sent an email with a link to access your forms for completion.  

Your name will appear in the Profile form. Please provide your phone number and other requested information.  You may also choose to upload your resume at this time, however you can choose to upload this at a later time. Press Next.

Please provide all of your Health and Medicine, Healing and Wellbeing and/or Specialty professions.  Please be prepared to provide your license numbers and certificates.  Please press Next.

Please provide information on your education.  Verification of education and credentials in addition to background checks may be conducted by Universal Background Screening.  Please press Next. 

Professional references are required to submit your forms. Please provide a name and phone number for two references. 

The final form is the consent form. Please check the consent boxes and submit your forms.
