For Provider Silver and Gold Members:

After you complete your KnoWEwell Provider Profile, you will answer a few questions to create your Organization Profile. The more complete your organization profile, the more people can learn about your practice and contact you.

If will be helpful to you if you have the following information on hand before you begin answering the Organization Profile questions:
  1. Business Name
  2. Organization ID # or EIN #
  3. Website URL
  4. Business Insurance Information, if applicable
