Not all fields are required but a more complete profile will help build a full picture of the Provider's practice for potential patients. Members will search for many of the attributes built into the Provider Practice Profile, and having as many fields as possible populated will ensure greater success in matching with potential members.
You can save your completed information and finish at a later time by pressing SAVE. You will receive an email with a link to your saved document. Take a look at the following forms so you can gather the needed information before you begin. Welcome to the KnoWEwell Community!

If will be helpful to you if you have the following information on hand before you begin answering the Organization Profile questions:
- Business Name
- Organization ID # or EIN #
- Website URL
- Business Insurance Information, if applicable
If you did not complete your organization profile immediately following the completion of your professional file, follow these first 3 steps; otherwise, skip to step 4.
- Login to the community using the username and password from when you created your account. If you have trouble logging in, click here for help.
- Click the button on the top navigation bar, Upgrade
Step 2: Click Organization Form (see image)
Step 3: If you currently work with an organization that is already in the community, you would answer Yes, and try to find that community. (Most likely, you'll select No.)
After you have selected that organization, it will ask if there is another organization you would like to add. If you have your own business that you would like to have displayed as part of your profile, you would answer
Step 4: Complete the remainder of the form with as much detail as possible.
